This application for a Homestretch Greyhound Rescue and Adoption greyhound dog is subject to the evaluation of the Homestretch Director . Please understand that a specific dog selected or any greyhound may not be appropriate for your situation. It is the Homestretch goal to find the right home for each greyhound. You must be 18 years of age to apply for a dog.

To notify Homestretch Greyhound immediately in the event my greyhound becomes lost or stolen.

The greyhound will be housed inside our home, as a member of the family.

To keep my greyhound on a secure, 6’ or shorter leash with a martingale style collar, when it is not in a completely enclosed (fenced) area.

The greyhound will not be used for racing or any other moneymaking endeavors.

To provide the necessary funds to care for my greyhounds daily needs and any health or medical care that may be required.

To inform Homestretch Greyhound of any change of address or phone number (for contact purposes in case the dog becomes lost).

To keep a collar on my greyhound with my contact information and Homestretch tag at all times.

In event that problems develop that prevent me from keeping the greyhound, I will contact Homestretch immediately and shall never give, sell or surrender the greyhound to any individual, rescue group or animal control facility or shelter other than Homestretch Greyhound and its authorized representatives.

I/we certify the above information is accurate and complete to the best of my/our knowledge. I/we authorize Homestretch Greyhound Rescue and Adoption to contact and verify each of the above employers, landlord and veterinarian.
